2018-2019 Applications

The CHS Prairie Lakes Community Funds program was established as a way to provide project funding for local schools, agricultural groups, and community organizations, along with graduating high school senior scholarships. Each year, the Community Funds program runs quarterly, with the first application deadline occurring in October. Please see below for descriptions of each award category and the online application. Please note that paper applications will no longer be accepted for this program.

Elementary & Secondary Education Grant

2018-2019 Elementary & Secondary Grant Application

To help ensure a strong future in rural America, we will be awarding multiple grants throughout the 2018-2019 school year. The grant amounts awarded may range from $50 to $1000 and are based on information provided in application and are decided by the Funds Committee. Payment of the grant will be made directly to the school/department as indicated on this application.

Recipients must provide proof of purchase showing their purchases/expenses by August 15, 2019 before receiving the payment. This grant program has multiple application deadlines, as stated below under Grant Period. All applicants must submit a W-9 (2018 version) with application.

Application Qualifications
Qualifying Public School Districts: Alexandria, Belgrade-Brooten-Elrosa, Browerville, Long Prairie-Grey Eagle, Melrose, Menahga, Minnewaska, Morris, Nevis, Osakis, Park Rapids, Paynesville, Sauk Centre, Sebeka, West Central Area and surrounding schools.

Support is towards the advancement and development of resources, equipment, and supplies for use in the classroom. Funding provided at the local district or classroom level.

Application Process
To apply, answer the application questions below, provide required file (W-9), and submit no later than the deadlines listed below under Grant Period. Instructors and/or principals may have multiple applications during each deadline period and are encouraged to have multiple requests throughout the year.

All grant applications are reviewed by the Funds Committee and applicants will be notified of the results of the meeting following application review.

High School Senior Scholarships

2019 High School Senior Scholarship Application

Ensuring a strong future for agriculture is the primary goal of CHS Prairie Lakes scholarships. We will be awarding several scholarships to high school seniors in 2019, with amounts ranging from $250 to $1250 based on information provided in the application, as decided by the scholarship committee. Payment of scholarship monies will be made jointly to the scholarship recipient and their college or university as indicated on this application. Recipients must prove their enrollment before August 15, 2019 to receive funds.
*If awarded a scholarship over $600, you will be required to submit a W-9 before payment.*

In-office application deadline for this program is January 1, 2019 at 5 pm.

Applicant Qualifications:

  • Resident of the CHS Prairie Lakes trade area
  • Graduating high school senior
  • GPA of 3.0 or better
  • Must have a designated major
  • Must submit completed application (applications with missing information will not be considered)

Selection process:
Applicant selection will be based on academic achievement, leadership characteristics, community involvement, and essay responses. Recipients are selected from the applying pool of applicants by the scholarship committee. Special consideration is given to children of CHS Prairie Lakes’ member-owners.

Agricultural Education Grant

2018-2019 Agricultural Education Grant Application

To help ensure a strong future in rural America through instruction, CHS Prairie Lakes will be awarding multiple grants during the year. The grant amounts awarded may range from $50 to $1000 and will be based on information provided in application and is decided by the Funds Committee.

Payment of the grant will be made directly to the 4-H club, Extension County, FFA Chapter, or educational group as indicated on this application. Recipients will have to prove their purchases/expenses by August 15, 2019 before receiving the payment. This grant program has multiple application deadlines, as stated below under Grant Period. All applicants must submit a W-9 (December 2017 version) with application.

Applicant Qualifications:
Qualifying counties: Becker, Douglas, Grant, Hubbard, Pope, Sebeka, Stearns, Stevens, Todd, Wadena
Qualifying Public School Districts: Alexandria, Belgrade-Brooten-Elrosa, Browerville, Cyrus, Long Prairie-Grey Eagle, Melrose, Menahga, Minnewaska, Morris, Nevis, Osakis, Park Rapids, Paynesville, Sauk Centre, Sebeka, West Central Area, and surrounding schools.

Support is targeted to assisting organizations in developing agriculture resources for their members or for hosting an educational event focused in agriculture. Funding is provided at the local, district or classroom level.

Returning Value to Local Communities Grant

2018-2019 Returning Value to Our Local Communities

The Returning Value to Our Local Communities grant is offered to help ensure a strong future in rural America. CHS Prairie Lakes will be awarding multiple grants throughout our fiscal year of September 1, 2018 – August 31, 2019 to communities and their organizations. The grant amounts given may range from $100 to $1000 and are based on information provided on the application; funding for this grant is decided by the Funds Committee.

Payment of the grant will be made directly to the organization as indicated on this application. All awarded funds must be used for direct project or program costs. Recipients must prove their purchases/expenses by providing a receipt or proof of purchase before August 15, 2019. This grant program has multiple application deadlines, as stated below under Grant Period. All applicants must submit a W-9 (December 2017 version) with application.

Applicant Qualifications:
Applicant must reside or exist within these qualifying communities and their surrounding areas: Alexandria, Belgrade, Brooten, Browerville, Cyrus, Elrosa, Farewell, Glenwood, Greenwald, Grey Eagle, Hoffman, Kensington, Long Prairie, Lowry, Melrose, Menahga, Morris, Osakis, Park Rapids, Paynesville, Sauk Centre, Sebeka, Sedan, Starbuck, Villard Westport, and surrounding areas.

Support is targeted to programs or initiatives that enhance the quality of life for farmers and rural communities. Grants are restricted to the following uses: Project or program support, Technical assistance, General operating support, and Equipment.
